Scheduling An Appointment
Once You've received a referral from your Doctor:
Scheduling
1. Call one our conveneint locations to schedule your appointment.
2. Please read the information carefully that is written on the referral form prepared by your doctor. We will do our best to provide an accurate fee estimate and an approximate length of time for your appointment.
At Your Visit
1. Please bring the referral form that your doctor has given to you, to your appointment.
2. Fill out your Patient Consent Form early! Bring it to your appointment as well to save yourself time. At the time of your visit, payment will be due. We do accept checks, cash, credit cards and Care Credit for your convenience. Insurance claim submission is usually handled by your medical/dental provider with your superbill information. You will receive a superbill receipt to submit to your insurance company for reimbursement per your individual policy.
Your images will be sent directly to your referring doctor within a few days, many times within a few hours of your visit.
General Guidelines
*Please try to arrive 5 minutes early
*Discuss any special needs for physical limitations with us in advance.
*Remove all jewelry from head and neck.
*Brush your teeth!
